How is your customer running an american refrigerator on the UK supply voltage? When you tested for 'continuity to earth' what test instrument were you using? If it was a normal multimeter you possibly won't see the insulation fault because it only uses a very lw voltage to perform the test. You may need to borrow a 'Mega' tester to get a meaningful reading. If the appliance is testing less than 10000ohms (1Mohm) at 250volts then it will trip a 30mA RCD or RCBO and you must localise the fault.

Another trick to remember with an RCD fault is that you must completely disconnect any component (both live and neutral sides) to be able to rule it out as the problem item.
