Hi all,
each time we do an intervention on a unit we have to wright a "leakproof " form/paper.
this cind of paper has bin created by a governement proved organisation.
i want to now how the form looks like in other country's.

Ice, you can use whatever form you like, only the important items must be found on this paper, it even can be a hand written A4 paper or even on a piece of toilet paper
It's better to keep your mouth shut and give the impression that you're stupid than to open it and remove all doubt.

In the UK its up to the different companies to sort out what commissioning/ service sheets they have. Some do not have any, others design their own and some use copies of the manufacturers.
So for pressure test records the only answer is your lucky to see it at all.Quidvis Recte Factum Quamvis Humile Praeclarum.
