    How do I determine the COP of a 12 year old centrifugal chiller ?

    Plot the running parameters on to a Mollier Chart and do the maths.

    Sorry, but, there is not really any other way.

    Measure flow at evaporator and delta T of water/glycol and calculate cooling capacity from that.
    Measure chiller power consumption and divide calculated cooling capacity with that, and you got your actual chiller COP.
