why in the name of all logic are they not obliged to fit leak detectors, even a lack of oxygen detector could have saved them.

Er! It's a boat, usually sharing machine room plant spaces.
Deisel and other gasses like exhaust fumes etc.
Can make leak detection a nightmare.
Not an easily resolved problem on a boat.
I totally agree with the oxygen monitor idea though Al!

One person had woken to smell..... It means he was in cabin. Hopefully, he didn't sleep in engine room. It can happen if engine room exhaust fan blows air to inlet of cabin supply fan. This is bad design.
