Has anyone had practical experience with installing & maintaining those systems? Are they difficult to pipe into the refrigerant loop?

Does anyone do a R-134a system?

Yes, I have installed many units like this one.
There are a few things you have to bare in mined before you start.

1. Quality of water. You will need to add a unit that takes care of the scale in the water.
2. make sure that the head pressure will not decrease too much.
3. Re tune the TEV.
4. A receiver must be used so if it is not present, you will have to add one.
5. Never run such a system without water.
6. The unit should always be above the condenser or it will collect the oil.

Another way is to heat the water with PHE, run the water to AHU and heat incoming fresh air in the winter.
